  • You're age 65 or older and a U.S. citizen or permanent legal resident for at least 5 years
  • You're under 65 with a qualifying disability and have received SSDI for 24 months
  • You have ALS (Lou Gehrig's disease) — eligible immediately upon diagnosis
  • You have End-Stage Renal Disease (ESRD) requiring dialysis or a transplant
